Your treatment plan will include a breakdown of all applicable (pre)- estimated fees and we will inform you of all costs before treatment is administered.  

Our office accepts payments by cash, checks, debit and Visa, MasterCard, American Express and Discover credit cards.

In addition to those forms of payment we are pleased to offer financing options through Care Credit and Wells Fargo Health Advantage.

Our office is committed to helping you maximize your  dental insurance benefits. Because insurance policies can vary drastically, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.

When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan please be sure to let us know.  A copy of a drivers license will also be required so we can validate subscriber-ship. 

If an insurance referral is necessary, please bring the completed form with you to your appointment.

We are a preferred contracted provider for Delta Dental & CIGNA and can submit claims to all carriers on your behalf ( out of network benefits may differ from contracted benefits, please call your carrier for plan specific details).

Although we will fully attempt to help you receive your insurance benefits; please remember, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within a reasonable time frame.
