We finally ditched the paper forms!
Smiles for the Future Pediatric Dentistry, Orthodontics and Adult Dentistry recently installed brand new patient communication software to aid in the completion of all digital patient forms, such as the medical history and registration forms. This decision was intended to simplify the overall process, maintain critical up to date patient information and lessen the redundancy of completing forms.
All New and Established patients will be sent a digital forms link embedded within the appointment and save the date reminders instructing to complete the initial series at least one time with timed updates as needed.
Once the digital forms are completed and submitted, the information will be securely uploaded into your or your children’s digital charts at Smiles.
Future appointments thereafter, the patient will receive the live link in the appointment reminder inviting them to “Check In” and click on the forms link to review, update or confirm the data on file.
AVOID DELAYS! Completed forms are REQUIRED by your provider at the start of your dental visit.
We no longer have paper forms available as a option at the front desk.
